Lightning Lacrosse Elite takes safety very seriously. We ask that each player have their own insurance. We supplement that insurance with US Lacrosse Event insurance. We ask that each participant fill out and bring this health/release form with them to the camp. Please do not mail this to us. Please bring it with you to the first day of camp.
Each camper must also fill and bring with them, this health/release form. THIS IS MANDATORY!
